Why AI pilots die before production (and the fix)
MIT's 2025 study of enterprise generative AI found that 95 percent of pilots delivered no measurable return on the profit and loss account. Gartner has since predicted that over 40 percent of agentic AI projects will be cancelled outright by the end of 2027, on cost, unclear value or missing risk controls. Neither figure is about the model. Both are about what happens to a pilot after the demo goes well.
The pilot was never the hard part
A demo proves a model can do the task in a clean room, with clean data, with someone technical driving. Production asks something different: can it do the task inside the business, fed by the business's actual data, run by people who did not build it, every day, without someone watching. MIT's researchers were blunt about the gap. Most tools sat beside the workflow instead of inside it. They could not hold context between sessions, could not adapt when the process around them changed, and could not survive contact with a system that already existed. The pilot worked. The business did not change shape to keep it.
Four ways a pilot dies quietly
The causes repeat across every failed pilot we have been called in to look at, and they rarely make it into the post-mortem slide.
No one owns it past launch. A pilot gets built, presented, applauded, and handed to whoever happens to be free. Six weeks later nobody remembers why the automation stopped running, because nobody's job depends on it running. We wrote about this pattern in full in who should own AI in your company.
Success was never defined. "See if it helps" is not a target. Without a number agreed before the build starts, there is nothing to measure against at day 30, and no way to tell a stalled pilot from a slow-burning success. Gartner's own data shows most agentic AI projects today are early experiments driven by hype rather than a costed business case, which is exactly the condition that gets a project cancelled once the budget is reviewed.
It was built to sit next to the work, not inside it. A tool your team has to remember to open is a tool your team stops opening. The pilots that survive are wired into the systems people already use, so the automation runs whether or not anyone thinks about it that day. That is the whole argument behind coordinated systems over isolated tools.
Governance arrived after the pilot, not before it. Data residency, access controls and a compliance sign-off are cheap to design in at the start and expensive to retrofit once a pilot has already touched customer data. Most stalled pilots hit this wall in month two, when IT or legal asks a question nobody thought to answer in month one.
The fix is not a better model
Every cause above is a management failure dressed up as a technology problem, which is good news: it means the fix does not require waiting for a better model. It requires treating the pilot as a piece of the business from day one, not a demonstration that gets promoted into one later. That means naming an owner before a line of code is written, writing down what the pilot has to be worth before it goes live, building it to plug into the tools your team already has open, and clearing data and access questions in week one rather than week eight.
This is the shape of every pilot we run. It is a deliberately narrow window to prove value in, and narrow is the point: a pilot with nowhere to hide either earns its place in the business or it does not, and you find out in a month rather than a year.
What this costs to get right
A properly scoped pilot sprint runs GBP 3,000 to 8,000 depending on the systems involved, priced against the value it is meant to unlock rather than the hours it takes. That is not a discount on the failure rate above. It is the cost of doing the unglamorous parts, ownership, measurement, integration, governance, that the statistics say most pilots skip.
